What is the right Excedrin Migraine dosage?
For adults, the labeled dose is 2 caplets with a glass of water. Do not take more than 2 caplets in 24 hours unless a doctor directs you to. Under 18, ask a doctor first. If the first dose does not hold, or symptoms deepen, that is a clinician question — not a second handful.

Three active ingredients in one formula
Per caplet: acetaminophen 250 mg and aspirin 250 mg for pain, caffeine 65 mg as the aid. The combination is useful when you already know your pattern and want an OTC option that stays on the shelf until you need it. Glance at the rest of the day too — other acetaminophen, other aspirin or NSAIDs, other caffeine — so nothing doubles by accident.
What the papers actually say
Three double-blind trials pooled by Lipton and colleagues in 1998 tested this same combination (two tablets: 500 / 500 / 130 mg) against placebo. Among 1,220 people treated for one attack, pain was mild or gone at two hours in 59.3% on the combination versus 32.8% on placebo. At six hours, 50.8% were pain-free versus 23.5%; pain was mild or gone in 79% versus 52%. Light, sound, nausea, and function eased more than placebo as well.

That is a real effect for many attacks that still belong on an OTC label. The trials did not include people who usually needed bed rest or who vomited with 20% or more of their migraines. One good study day is also not the same thing as a month of caplets.
Combination analgesics that include caffeine sit in the moderate-risk group for medication-overuse headache. The Excedrin Migraine label notes that headaches may worsen if the product is used 10 or more days per month. Mayo Clinic groups these caffeine combination pills the same way and suggests keeping combination pain relievers under about nine days a month when the month allows it.

Use it only when the label fits
This product is for migraine. Not every severe head belongs in self-care first. Ask a doctor before use if you have never had migraine diagnosed, if this headache is different from your usual ones, if it is the worst of your life, if there is fever with a stiff neck, if headaches are daily, if vomiting comes with the attack, or if you need bed rest.
A quiet check before you swallow:
- You are an adult, or you have asked a doctor if under 18.
- You are not taking another acetaminophen product today.
- You are not stacking other aspirin, NSAIDs, or fever reducers unless a clinician said to.
- You have a sense of today’s caffeine — coffee, tea, energy drinks, chocolate, other medicines.
- You have read the Drug Facts, including liver, stomach-bleeding, heart or kidney, asthma, and allergy notes that may apply to you.
- This is not already a heavy caplet month.
Aspirin products carry a Reye’s-syndrome warning for children and teenagers who have or are recovering from chickenpox or flu-like symptoms. Pregnancy — especially from 20 weeks — is a clinician conversation.

Important safety guidance
Stay with the labeled maximum unless your doctor writes a different one. Severe liver damage may occur if more than 2 caplets are taken in 24 hours, if it is used with other acetaminophen drugs, or if someone has 3 or more alcoholic drinks every day while using it. Aspirin is an NSAID: stomach-bleeding risk is higher at 60 and older, and with ulcers, bleeding problems, blood thinners, steroids, other NSAIDs, alcohol, or longer-than-directed use.
